I have an 80w universal ac adapter that I used with an older laptop. My current laptop requires a 90w power supply, (19V at 4.74A). My question is this. Since my current ac adapter can only handle 80w and has adjustable voltage, is it "better" for the computer to have the proper voltage (19V) at a reduced amperage (4.2A) OR something closer to the proper Amperage (4.5) at a slightly lower voltage (18V).

You do not say how old the battery is, but it sounds like a bad cell too me. This can happen from not charging the battery properly (typically from under charging). You can rebuild the battery (replace the bad cell), but this will only delay the inevitable, you need a new battery.
